Access to Medical Records
We respect your right to private and keep all your health information confidential and secure. It is important that the practice keeps accurate and up-to-date records about your health and treatment information. We have a fully computerised medical record system which means information about your healthcare is held on a secure server. You have the right to know what information we hold about you. Please ask the receptionist if you wish to see or obtain a copy of your record. If you are referred to a hospital specialist you are welcome to request a copy of the referral letter. NHS Connecting for Health has developed the National Care Records Service, which is creating a Summary Care Record (SCR) for each individual patient to be held centrally. At this pilot stage the record is limited to current medication, known allergies and adverse reaction. Eventually the aim is to enable the healthcare staff jointly involved in your care, such as your GP and your hospital consultant, to easily access and share information through this national record. You have the right to withdraw your consent to information being shared. In future you will be able to access a summary of your record online at www.healthspace.nhs.uk.
In certain circumstances information may be withheld from patients seeking access under the Data Protection Act. This would normally be when a doctor judges that such information might be harmful to the patient, or where it concerns a third party.
Under the data protection act 1998, In case you as a Carer or patient make a complaint the practice may need to provide information from your medical records about you as the patient, and treatment you have received, to insurers or legal advisers.
